Registration & Rates
There will be a yearly registration fee of $15.00 per child or $45.00 per family that is non-refundable. Registration requires a completed enrollment and medical form. The medical form must be completed within 30 days of your first visit at Kids Zone Flexible Childcare. Follow the links below to print each form. Your physician can fax the medical form directly to us at 614-899-6335.

3 Children: $170.00 for a ten hour punch card

*Snack is included in hourly and weekly price
*Snack is served twice daily with 2 food groups each
